Registration Forms:

MYA Basketball Sign-Ups are OVER!

Now you need to submit a LATE registration.

  1. Get a MYA Basketball Web Account at Sign Up for Web Account
  2. Follow the "Fill Out Player Registration Form" link after you're logged in to send your data to MYA Basketball
  3. Print the filled out form to get hard copies of your registration and medical form
  4. Take the filled out forms with a check to the drop box at the MYA Building by Sat Oct 8 to pay and finalize your registration.
  5. If you can't get the forms there by Saturday, contact your coordinator and bring the forms to tryouts Oct 11 or 13.

If sometime during the season, your player gets hurt playing MYA Basketball, you may need to see a doctor or hospital. MYA Basketball has secondary insurance that picks up certain costs, after a $100 deductible has been satisfied. If you need to use it, download the Insurance Forms to apply for the insurance.
